NOTE: I have received 2 game score and 2 Rep Info forms that were blank. I had no way to determine who sent them. I have spent several hours re-doing the code so the senders name will be in the header and the sender will also get an email copy of the form they sent to me. This way you can check to see what was submitted, and I can insure that I will know who is sending in the forms. I beleive that the forms will submit properly now. Just a reminder that all home teams are responsible for sending in game scores using the form below. They must be submitted within 48 hours of completion of the game. The League will implement fines if you fail to report on time.
