GLGHL Score Reporting Form

New for 2009-2010 - Misconduct and Match Penalty Reporting Form - Must be filled out and sent within 24 hrs. and must include your teams next 3 scheduled games. Click Here. Home team's are responsible for scoresheets and reporting scores. Please use this form to report all league games results promptly. All fields on this form are required. You must include the game number and the date game was played as well. Please list the number of penalties and total penalty minutes as well. Double minors count as two penalties. Also, don't forget to mail or fax your scoresheets to your division commissioner within 72 hours. You must report all game misconducts and match penalties immediately on this form and to your division commisioner. Please list the reason (ruling from referee) in the "Report Game Misconducts" box. Also, when a suspension is served, please list that as well. Thank you for your cooperation. Any questions email me, The Webmaster.
